The last two numbers indicate the chain's inner width. So for a 428 chain, the "28" means 2.8/8 inch width. Getting the size right is crucial because an improperly sized chain won't just perform poorly - it can actually be dangerous .
Most quality chains are made from alloy steel and come with various surface treatments like zinc plating or chrome plating for corrosion resistance.
